After IO is signed, this form will be used to capture deals that require activation.

Please note:

1. For deals signed prior to 8/15, the earliest start date for deal activation is 8/18/2025, 11AM CT

2.After 8/18, deals are activated within 24 hours from submission

3.This form will be sunset once deals can be entered directly by Sales team.
